Return Your Paperwork
Return your Paperwork -You have several options for completing your new patient paperwork. We can mail the forms to your home, or you may complete them online. The process typically takes 10–15 minutes. Promptly returning your paperwork helps us support you as efficiently as possible.

Schedule Your Intake
Schedule your Intake- Once your new patient paperwork is received, our referral department will contact you to schedule your intake. After your appointment is scheduled, all that’s left to do is attend—either virtually or in person.
